NOTICE: Computer containing personal information lost

On February 26, 2014, the Ohio Department of Mental Health and Addiction Services (OhioMHAS)

discovered an event that may have involved an unauthorized access or disclosure of protected personal

information of up to 235 individuals who received community linkage services during the time period

from February, 2013 to February, 2014 from the following Department of Rehabilitation and Corrections

or Department of Youth Services facilities: Ohio Reformatory for Women; Lebanon Correctional

Institution; Warren Correctional Institution; Circleville Juvenile Correctional Facility; and, Scioto Juvenile

Correctional Facility.


This incident occurred on or about February 26, 2014. A laptop computer was stolen from the vehicle of

a department employee. Access to the information on the computer was protected with a user name

and password, but the contents of the computer were not encrypted. The information which was

stored on the computer includes name, date of birth, address, institution assignment and number,

contact information, date of release, diagnoses, medications, psycho‐social history, agency referral, and

insurance information.


OhioMHAS has investigated the source and extent of this unauthorized access or disclosure, and has

reported the theft to the Columbus Police Department and to the Ohio State Highway Patrol.

If you believe that you may be impacted by this incident, please contact the department. In order to

protect affected individuals from any potential harm which may come from this data breach, the

department has arranged to have AllClear PLUS identity protection available to those individuals whose

information may be impacted, at no cost to the individual, for one year.

by mail, using the attached form, anytime within the ninety days following the date of this notice. To

enroll, you will need a redemption code. You must contact the department at the number or address

listed below to obtain the redemption code. Please note: AllClear ID may require additional steps by

you in order to activate your phone alerts.
