ACF/AFoH Incentive Program
OhioMHAS is offering the Adult Care Facility/Adult Foster Home (ACF / AFoH) Incentive Program for State Fiscal Year 2017 (SFY17). The program began July 1, 2016 and will go through June 30, 2017. The ACF / AFoH Incentive Program provides resources to operators to enhance the quality of care of licensed homes which provide housing and supports to individuals with mental health or substance use disorders and support their efforts towards meeting Home and Community-Based Services (HCBS) requirements. In order to be considered an HCBS-eligible setting, the residential setting must be integrated and support full access of individuals to the greater community, including opportunities to engage in community life, control personal resources, and receive services in the community with the same degree of accessibility as individuals without disabilities.

SFY 2017 Quarter 1 and 2 Update:

The payment for the ACF Quarter 1 and 2 Incentive has been released and home operators should have already received payment. If you have not received payment for Quarters 1 and 2, please email ACF-AFoHIncentive@mha.ohio.gov


SFY 2017 Quarter 3 and 4 Update:

 

The deadline has passed for accepting ACF Incentive Quarter 3 & 4 applications. The payment is scheduled for October 2017. The funding for the incentive program will be fully spent. Please see the following letter which explains in detail.

What is the Application Process?

If you have arranged electronic fund transfer (EFT) you can check the status of your payment on the OAKS EFT Remittance Lookup website at http://remitlookup.obm.ohio.gov/. In the Vendor ID box enter your 10 digit OAKS Vendor ID (including leading zeros). In the Key# box enter your 5 digit zip code associated with your Vendor ID. Click on the box next to "Date Range." Type in the beginning date (07/01/2015) and in the "to" box the ending date of your search (06/30/2016). Click submit and all payments issued under your Vendor ID will appear.

If you previously participated in the ACF Incentive and received funds (via direct deposit or hard copy check) you have a valid OAKS Vendor # and do not need to apply for another.  If you do not have an OAKS Vendor number then you cannot participate in the program.  Following is information to obtain an OAKS Vendor number.  The forms noted below may be accessed at: Ohio Shared Services Suppliers Forms Site.

  • Dear State of Ohio Potential Vendor Letter (OBM-7502); print and read for additional clarification on this portion of the process

 The following forms are required to be completed and submitted directly to Ohio Services:      

  •   Vendor Information Form (OBM-5657) (to obtain the OAKS Vendor#)
  •   IRS Form W-9

The form noted below is only required if you would like directly deposit (Optional, but highly recommended)

  •  Authorization Agreement for Direct Deposit of EFT Payments (OBM-4310) (please include a copy of a voided check or a bank letter, otherwise your request cannot be processed)

The above mentioned forms are to be sent to Ohio Shared Services for processing.
